6. Taxes and contributions:

  • You pay annually insurance premiums in a fixed amount, in 2017 it is 23,400 rubles. ( Pension Fund) + 4590 rub. (medical insurance) = 27990 rubles. Insurance premiums are indexed annually (for comparison: in 2016, the total amount of insurance premiums was 23153 rubles; in 2015 - 22261 rubles, in 2014 - 20728 rubles).   • If your annual income exceeds 300,000 rubles, you pay additional insurance premiums - 1% of the excess amount.

4. CONCLUDING A WORK AGREEMENT WITH A PSYCHOLOGIST

When deciding on joint work at the end of the second meeting, the psychologist and the client conclude a cooperation agreement. The preliminary duration of the work can be a course of 5, 10 or 15 meetings, at the last of which a decision is made to continue or complete the work.

5. FORMAT OF WORK

Session, usually happens once a week at the time agreed by the psychologist and the client. Meetings may be more frequent if necessary.

A specific time is assigned to the client. The specialist undertakes not to occupy the time assigned to the client by other clients, except in cases where the client cancels the appointment. In exceptional cases, by mutual agreement, the meeting time can be changed.

Any changes, such as payment, time, frequency of meetings, format of work are discussed between the client and the psychologist during the conclusion or revision of the contract, or, if necessary, at the initiative of the client or psychologist.

6. PAYMENT FOR PSYCHOLOGICAL SERVICES

Method of payment - cash after the consultation. If you want to use other methods of payment for services, you need to inform about it during the appointment for a consultation in order to receive the details.

The amount of payment may change annually, but not more than twice a year, usually in autumn and spring. The increase cannot exceed 25% of the current consultation price. The psychologist undertakes to report these changes at least a month in advance.

Each session is paid by the client in a form convenient for him: before the start, after the end of the session, payment in advance for several sessions is possible. A debt session will take place in exceptional cases.

At the second meeting, the client pays the amount of 100% of the cost of the consultation, which will be collateral in case of non-compliance with the terms of the contract. If the client fulfills all financial obligations, then the last meeting takes place at the expense of the security deposit (without payment).

7. POSTPONE OR CANCEL THE CONSULTATION

The client undertakes to notify the specialist about the cancellation of the meeting at least 24 hours in advance (the day before), otherwise, the client pays for the time, except in cases of force majeure. The specialist, for his part, in case of cancellation of the appointment less than 24 hours in advance, next meeting conducts free of charge, except in cases of force majeure.

Whether the impossibility of attending the session is a force majeure circumstance, the client or specialist evaluates for himself. Force majeure circumstances may be considered the physical impossibility of attending the session due to: natural Disasters, accidents, illness, as well as other extraordinary circumstances, which are discussed at a subsequent session.

If the client decides to terminate the consultations unilaterally, then payment is made from the security deposit.

If the client is late for the consultation, its time is reduced by the time of delay. For example, if a client arrives at a meeting at 6:15 pm instead of 6:00 pm, then the consultation still lasts until 6:55 pm, and its end time is not shifted.

8. OBLIGATIONS OF THE PSYCHOLOGIST AND THE CLIENT

The psychologist is responsible for being attentive, observing what is happening with the client and informing him of his observations, opinions, versions and hypotheses. For everything else (conclusions, decisions and changes in your life), the client is responsible.

In order to benefit from the meeting, the client needs to get involved in the work as much as possible, in exercises, in discussion.

At the same time, the client can stop statements, questions or suggestions addressed to him.

For the duration of the session both parties undertake to provide working conditions(do not get distracted by other things, telephone conversations, chatter about extraneous or personal matters).

During the whole time while working with a psychologist, the client leaves the final decision, for example: marriage, divorce, changes in career, place of residence, etc. for the period after the end of the consultation or discusses these issues with a specialist, because such decisions can be made under the influence of affect or exacerbation of the psychological state, which is possible when working with traumas.

9. ETHICAL RULES IN WORKING WITH A PSYCHOLOGIST

No type of violence allowed(economic, physical, sexual, mental), abusive acts, and the use of confidential information without the consent of both parties.

For the duration of the contract changes are not allowed social roles between client and psychologist. In particular, it is not allowed to enter into friendship, love, family and business relations with a psychologist or participate in joint collective activities that are not part of psychological work.

Any information provided by the client to the psychologist is confidential. and is not carried out beyond the limits of communication between the client and the psychologist. The psychologist undertakes to keep the very fact of seeking psychological help a secret, if necessary.

Preservation or violation of confidentiality by the client remains at his discretion.

Nothing the client says will be used against him.

Sometimes it is useful to discuss the case with other specialists (supervisor, psychiatrist or other doctor) for work. 10. DURING WORK WITH A PSYCHOLOGIST IT IS FORBIDDEN

It is impossible to conduct a lesson in a state of alcoholic intoxication (even light) or under the influence of substances that change consciousness, for example, drugs or strong sedatives, unless they are prescribed to the client by the attending physician for constant use. Substances that clarify and sharpen attention without distorting it are allowed, such as tea, coffee, nicotine, nootropics. Psychological work in a state of altered consciousness is not safe.

11. COMPLETING THE WORK WITH THE PSYCHOLOGIST

The client has the right to interrupt work at any time, taking a break in work for any period or permanently. In cases where a decision is made to interrupt work, you need to attend another 1-2 sessions in order to psychologically complete the work.

I.General provisions.

1. In accordance with these Rules, the Contractor (IP Yadrishnikova E.N.) undertakes to provide consumers (clients) with services for organizing psychological correction classes (family correction, game correction, neuropsychological correction, special correction, psycho-gymnastics, autogenic training, etc.), and the Customer undertakes to pay for the services provided under the terms of this agreement, as well as to facilitate their provision.

2. Under the psychological correction in these Rules is understood: the intellectual activity of a person who has a special (psychological) education in the analysis of psychological, behavioral problems and problems of the intellectual and emotional development of the client in order to further correct them with the available in science and practice psychology methods and tools.

3. Neuropsychological correction in these Rules is understood as: the intellectual activity of a person who has a special education in clinical psychology, including, among other special disciplines, such disciplines as neuropsychology, neuropsychology of childhood, clinical psychology in expert practice, pathopsychology, psychosomatics, psychology of stress conditions, etc., and which allows to analyze violations of the HMF for subsequent psychological qualification and correction of the defect detected in this case for the purpose of restorative and remedial training by methods that are used in psychological practice.

4. The services provided are not pedagogical or medical services and their provision does not require obtaining a license to carry out pedagogical or medical activities.

5. The place of provision of services is the premises of the Cabinet of psychological and neuropsychological correction, located in the residential premises of the contractor at the address: Sverdlovsk region, Verkhnyaya Pyshma, st. Ogneuporshchikov, house 9, owned by the Contractor. The use of residential premises for business activities is carried out in accordance with Art. 17 of the Housing Code of the Russian Federation.

6. The office of psychological and neuropsychological correction is not a group of temporary stay of children, a kindergarten, a circle, a section or other organization with a joint long-term stay of children or adult citizens. The Cabinet does not provide catering services, other social services, except for those specified in paragraph 2 of this section. The maximum time the client stays in the office during the provision of services is 1.5 hours.

7. The initial reception of the performer, as a rule, consists of the following stages: psychological analysis of the problem / observation, selection of working methods, determination of the approximate number of receptions (course), work with the identified problem, summing up and explaining homework. Repeated receptions (classes) consist of stages: discussion of the results of homework, work according to the plan in accordance with the chosen methodology, summing up the results of the reception and explaining the homework. Based on the individual needs of clients and the specifics of services, the stages of admission can be adjusted independently by the Contractor.

8. Primary and final receptions, at the request of the Customer, may be accompanied by a written opinion. Drawing up a conclusion based on the results of diagnosing existing problems and the results achieved is an independent service and is paid in accordance with the price list.

9. Classes with children are held only when accompanied by a specialist, with the exception of classes in a family group of parents-children or grandparents - parents-children according to child-parent relationships, group family receptions (classes) and an initial group reception, at which the presence of any of the parents is required .

10. Persons under the age of 14 receive services (classes) for themselves only when accompanied by a legal representative (parent, guardian, guardian) or a person acting on the basis of a written power of attorney issued on behalf of the legal representative. Persons aged 14-18 years old can receive services independently, subject to the written consent of the legal representative to receive the services and pay for them from the funds of the legal representative.

11. All the conditions of these Rules are binding both for the Customer and for the third party in whose favor the contract is concluded (the client), if the Customer and the client do not coincide in one person.

12. An integral part of these rules is the Price List and Rules of conduct for visitors to the office of psychological and neuropsychological correction.

13. These Rules are public contract within the meaning of Art. 426 of the Civil Code of the Russian Federation. The terms of the public contract are set the same for all consumers and are not subject to individual adjustment. These rules are the form of the contract accession within the meaning of Art. 428 of the Civil Code of the Russian Federation.

14. The text of the Rules is posted on the official website of the Cabinet in the public domain for all visitors, and is also issued personally for review before admission. Familiarization with these rules and the performance by the client of actions indicating the intention to receive the service (booking an appointment, attendance) by virtue of part 1 of article 160, part 3 of article 434, part 3 of article 438 of the Civil Code of the Russian Federation are recognized as a fact conclusion of a written contract with each of the clients.

II. Obligations of the contractor and customer (consumer) of services

1. The contractor is obliged:

As part of the initial appointment, provide information: about the alleged existing psychological problems, developmental problems; about the possible methods of psychological work in this case, the need to consult a doctor; about a possible change in the psychological state of the client and temporary discomforts of an emotional nature that may arise during the session; about the actions that need to be independently performed by the client in order to achieve and maintain the achieved results of psychological correction.

1.3. Determine for the Customer the required number of receptions in order to achieve the maximum effect from the classes.

1.4. If necessary, offer to undergo additional consultations and examinations with specialists of a psychological and pedagogical profile (special psychologists, speech pathologists, speech pathologists, educational psychologists) or in medical institutions in order to clarify the causes of the client's existing problems. The customer independently and under his own responsibility decides on the implementation or non-fulfillment of these recommendations of the contractor. If the Customer refuses to comply with these recommendations, the Contractor has the right to unilaterally refuse to execute the contract in order to avoid causing harm to the psychological health of the client.

1.5. Upon the fact of the rendered service, provide the Customer with a document confirming payment for the rendered service.

2. The contractor has the right:

2.1. Independently determine the schedule for the provision of services to clients, change it with the obligatory notification of the Customer in case of a change in the date of its acceptance. Independently make decisions about the need for a break in receptions, reducing their number and frequency in accordance with the individual characteristics of the client, psychological correction and work results for a particular client;

2.2. Cancel the appointment if the Customer is late for the appointment by more than 10 minutes. In case of repeated (more than 2 times) being late for an appointment - to set limits for the client on the possible time for an appointment, in case of failure to appear at an appointment without a 24-hour notice - to establish a reservation of the corresponding time with its advance payment; in the event that the Customer is unable to fulfill the meeting arrangement, he is obliged to notify the Contractor at least 24 hours in advance. If the Contractor does not receive such notification, the Customer is obliged to pay for the reception that he missed;

2.3. Do not provide services (refuse to provide them):

a) in the absence of free time in the schedule of client appointments;

b) if the Customer requires the provision of a service, arriving without an appointment, or after a break in work, due to illness or any other reason, without notifying the Contractor in advance of his intention to appear at the reception, without agreeing on the service and the time of its provision;

c) if the Customer is in a state of narcotic, toxic or other intoxication, a state of passion or arousal, determined by external signs, and also if for some reason he is not able to realize the meaning of his actions and manage them;

d) if the client's complaints (description of the problem) fall within the competence of the medical specialist;

e) to the Customer who violates the Rules of Conduct for Visitors in the Office of Psychological and Neuropsychological Correction (Health Psychology Office);

f) a customer who systematically fails to do homework;

g) if between the Contractor and the Customer and (or) the client (if they do not coincide in one person) relations of cooperation and trust have not objectively been built in view of the fact that the services are of a personal-confidential (fiduciary) nature, which has a direct impact on efficiency, effectiveness services provided.

2.4. In order to systematize information about the client, track the effectiveness of work, select and change the most effective methods of work, the Contractor has the right to accumulate, systematize and analyze material carriers of intermediate results of work with the client (including records, drawings, applications, etc. - more under the text "case of the client"). The client's case is the property of the Contractor.

3. The customer undertakes:

3.1. To be sincere and honest with yourself and the Contractor when describing the problems with which he addressed, as well as in the process of further psychological work; show interest in achieving a positive effect from the work; do your homework diligently. All of these obligations are natural obligations (obligations of "conscience"). The Contractor notifies the client that in case of failure to fulfill these obligations, no sanctions can be applied, however, the client bears the risk of not achieving the predicted result (positive effect from psychological correction).

3.2. . If it is impossible to attend the appointment for a good reason, notify the Contractor 24 hours in advance.

3.3. To conscientiously read these Rules, the rules of conduct in the Office of Psychological and Neuropsychological Correction, the price list and confirm the fact of familiarization with a personal signature in the Service Consent Log.

3.4. Inform the Contractor of information about existing neurological, mental or other diseases, the symptoms and course of which may affect the results of the services provided. The Contractor notifies the client that if the client fails to fulfill this obligation, the risk of negative health consequences is borne by the client himself.

3.5. Warn the Contractor if the course of receptions is not completed, and the client for any reason will be absent for a long time, or intends to move to another specialist.

4. The customer has the right:

4.1. Obtain reliable and complete information about the services provided within the framework of the Contractor's specialization (clauses 1.1., 1.2. section 2).

4.2. Choose the time of reception at the Contractor from the available free time in the schedule.

4.3. To invite in agreement with the Contractor at the reception of one relative, or another person whom he wishes and whose presence will be necessary to understand the essence of the problem. The presence of an escort does not cancel the Contractor's right to ask the escort to leave the office if the Contractor deems it necessary for the process of psychological correction or neuropsychological correction.

4.4. At the final appointment, upon request, receive the service "Professional opinion of a psychologist, clinical psychologist", having previously agreed and paid its cost in accordance with the price list.

III. Terms of service provision

1. The term for the provision of a specific psychological service is determined by the date and time of the Customer's appeal to the Contractor (Article 190 of the Civil Code of the Russian Federation).

2. The duration of a specific appointment can vary from 40 minutes to 1.5 hours, depending on whether the initial appointment or repeated, the client's age, methods of working with him.

3. The number of receptions is determined by the Contractor individually for each client, based on the problem voiced by the client, the chosen methods of work.

IV. The quality of the services provided. Warranty. Forecasts.

1. When providing services, the Contractor undertakes to act in good faith, carefully and prudently.

2. The service is considered to be rendered qualitatively if the Contractor complies with the standard of its provision generally accepted in practice on the territory of the Russian Federation (the corrective method is correctly chosen and followed). Achieving a specific expected result is not included in the subject of the obligation to provide services (Regulations of the Constitutional Court of the Russian Federation No. 1-P of January 23, 2007).

3. The services provided by the Contractor imply a possible change in the emotional and volitional state, behavior, perception of the client, which can in a certain way affect his life.

4. When providing services, the Contractor uses the methods listed in Appendix No. 1 to these Rules, as well as author's synthetic methods developed for the purpose of providing services to a specific client in the process of working with him.

5. No part of the author's synthetic methods (worksheets and tasks) obtained in the Cabinet of Psychological and Neuropsychological Correction can be reproduced, replicated and used for other purposes, in any form without the written permission of the author (Contractor). The transfer of worksheets, tasks to third parties who may voluntarily or unwittingly violate the confidentiality of the information contained in them is prohibited.

6. When providing services, the Contractor operates with the concept of "forecast" of changes in the personality of a particular client. Due to the intangible nature of the services, the Contractor does not establish guarantees for the occurrence of a certain result, its safety (Article 5 of the Law of the Russian Federation "On Protection of Consumer Rights"). The discrepancy between the results expected by the client (his legal representative) and the results obtained in the process of psychological and neuropsychological correction does not indicate a poor-quality provision of services.

7. A positive prognosis of psychological correction directly depends on the client's compliance with individual recommendations after receiving and doing homework.

V. Price of services and payment procedure

1. Prices for services are determined in the price list of the Contractor, which is an integral part of this agreement. Prices for services can change no more than 2 times a year and no more than 30% in total.

2. Payment for services can be made both after the provision of the service and before the start of its provision (paragraph 2 of article 37 of the Law of the Russian Federation "On Protection of Consumer Rights").

3. Payment for services is made by the Customer in rubles in cash. In the event of a shortage of funds to pay for the services rendered, the Customer personally writes a receipt indicating the amount of the debt and the obligation to pay for the services within three working days from the date of the receipt.

VI. Settlement of disputes

1. In the event that a misunderstanding arises between the Contractor and the Customer, if the Customer has questions for the Contractor, if the Customer wants to notify the Contractor about anything, he can send an Information Letter to the Contractor.

2. Information letter - not a claim document, sent to the Contractor if there is a desire to continue receiving services. An information letter can be written by hand, sent by e-mail, through the site form or stated as part of a telephone conversation. The information letter is considered within 3 working days, the result of the consideration can be communicated to the Customer by e-mail, in writing or by phone.

4. In the event of claims regarding the quality of the services provided, unwillingness to continue cooperation in the future, intention to protect interests based on the norms of the Law of the Russian Federation "On Protection of Consumer Rights", the Customer has the right to draw up an official claim.

5. The claim is made only in writing with the personal signature of the Customer, indicating the specific circumstances of the Contractor's failure to fulfill its obligations and the requirements that the Customer makes on the basis of the Law of the Russian Federation "On Protection of Consumer Rights". The claim is considered within 10 working days, the result of the consideration is drawn up in writing.

6. The Contractor does not accept claims for the quality of the services provided, which are based on negative conclusions issued by other specialists (psychologists).

VII. Responsibility of the parties

1. The Contractor is released from liability for violation of the deadlines for the completion of psychological correction (course), as well as for harm to health that occurred during the provision of services, if it is proved that the deadlines were violated through the fault of the client, and the harm arose as a result of the client’s violation of the recommendations given by the Contractor ( item 6 of article 28, item 5 of article 14 of the Law of the Russian Federation "On consumer protection", article 1098 of the Civil Code of the Russian Federation).

2. The Parties shall not be liable for failure to fulfill their obligations under this Agreement if the failure was caused by force majeure, that is, extraordinary circumstances that none of the parties could have foreseen and prevented in advance. The following are recognized as force majeure circumstances: decisions of municipal and state authorities, utility accidents related to the disconnection of heat, electricity, water.

The specialty of the performer is “Psychologist. Psychology teacher”, “Clinical psychologist” is a general humanitarian specialty, not a medical (medical) one.
