What is gout?

Gout is a type of rheumatic disease that affects the joints.

The disease progresses against the background of impaired metabolism and an increase in uric acid in the body, which is the final metabolite of cellular life.

Uric acid enters the blood plasma and is then excreted by the kidneys in the form of urine.

With an increase in its level, hyperuricemia occurs, in which acids are deposited in the joints.

This causes inflammation and aching pain, which intensifies over time.

In recent years, the number of cases of the disease has increased in the world: in European countries and the USA, the group of patients with goiter is about 6% of the total number of patients.

Why does the disease occur?

Initially, with gout, disturbances in metabolic processes occur, after which the disease affects the joints, especially the joints of the legs.

When the human body is working properly, uric acid is excreted in the urine.

If there are any disorders, including in metabolic processes, excess uric acid begins to crystallize and deposit in the joints. This leads to disruption of the joints, the appearance of pain and inflammation.

The main cause of the disease is an increased level of uric acid in the blood cells.

In turn, the increase in volume and accumulation of uric acid can be due to two factors:

The risk factors that provoke the disease are:

By what symptoms can the disease be recognized?

Gout can be recognized by two main symptoms:

  • on an attack of arthritis;
  • by the presence of gouty nodes.

The main clinical symptom of gout is attacks of acute arthritis, which:

  • appear suddenly (usually at night);
  • progress rapidly;
  • accompanied by headaches, the appearance of a tumor that acquires a burgundy hue, and an increase in body temperature up to 38 degrees.

Gout triggers also include:

  • alcohol abuse;
  • microtrauma resulting from long walking or wearing narrow shoes;
  • physical overload;
  • infections such as influenza or sore throats.

Another common symptom is gouty nodes.

They are nodules that appear near the ears and joints.

Such nodules affect the joints gradually, from which the pain intensifies.

With long-term observation of the patient, three main variants of the course of the disease can be identified:

Lung: attacks of arthritis appear no more than 2 times a year and affect 1-2 joints. X-rays do not reveal obvious signs of joint destruction.
Moderate: in this form, the frequency of attacks can reach 4 times a year and is accompanied by damage to 3-4 joints.
Heavy: the frequency of attacks per year is 5 or more. Here there is a multiple lesion of the limbs, which is characterized by severe destruction of the bone joints.

Clinical picture of the disease

The clinical picture of gout is as follows:

  1. There are sharp pains in the metatarsophalangeal joint (first). More often pain attacks disturb the patient at night.
  2. The metatarsophalangeal joint swells, peeling and hyperemia of the skin is observed.
  3. A few hours later, fever, chills may begin.
  4. The victim increases pain attacks, which cause immobility of the affected limbs.

Allocate forms of the first attacks:

Radiography

In the early stages of the disease, the X-ray method does not show characteristic changes in the joints.

In the chronic form of gout, radiography determines the destruction of the cartilaginous zones:

  • narrowing of the joint spaces due to the destruction of cartilage;
  • pronounced defects in bone tissue;
  • the process of erosion of the surface of the joints;
  • characteristic seals of soft tissues that border the joints;
  • in secondary osteoarthritis, marginal osteophytosis is added to the listed signs.

The most typical sign of gout is a pronounced defect in the bone tissue, which is formed in the first metatarsophalangeal joint and the articular zone of the hands.

There are 3 types of radiographic studies in the chronic form of gouty arthritis:
